Purpose
This survey will increase our understanding of the occurrence, frequency, and impact white mold disease has on soybean production and what management practices growers are willing and able to do or can do to combat this disease. Results from this survey will direct future research efforts towards building effective management strategies tailored for farm operations in each production region.
Instructions
The survey will take a maximum of 15-20 minutes, with questions getting more in depth as you progress. This should be completed in one sitting on a computer or laptop. Results are confidential and stored in a secured location. Your participation is voluntary and you may decide to stop at any time. You do not have to answer any questions that you do not want to answer.
Loss of confidentiality is a potential risk. However, all electronic records of research data will be stored in the Penn State provided database application, REDCap and PSU OneDrive. Only the researchers on this project have access to data. Data will be aggregated by county so that results will not be linked to specific individuals.
A summary of the data will be shared to growers through extension workshops and written articles through communication channels such as Field Crop News and Lancaster Farming.

You may choose to enter into a drawing to win a $50 Tractor Supply gift card by providing your email. If you are selected as a winner, you will be notified via email at the close of this survey. Your email address will not be associated with your answers to the survey questions.
